Online Registration Agreements

  • We are providing a list of some of the lengthier agreements that parents will be asked to "sign-off" while filling out the Online Registration Application. These are being provided as they will appear in the registration, or as links to documentation, for your convenience.

    We suggest reviewing this information before registering your students to help you move more quickly through your application. We hope you find this useful.

All Students - Release Agreement - Media, Field Trips, and Pesticide

  • Media
    Students occasionally have photographs or video taken of them engaged in school-sponsored activities. These activities may include, but not limited to, performing in a special program, a school play or athletic event, displaying samples of student work, or representing a particular instructional program. The school/district may use these images in various publications, including newspaper, school/district newsletters, school/district website, or any other district publication; the student may or may not be identified by name with their image. Parents who do not want their student's photograph/image or work to be published in print, video or on the Web should provide written notice indicating their request to the school on an annual basis before the beginning of the school year.

    Field Trips
    Field trips are taken throughout the year to enhance the curriculum. Teachers will send information home about the trips as they occur. Parents are sometimes asked to chaperone on the trips when additional chaperones are required. Parents will have to provide their own transportation when they attend as chaperones. Students must return to school with their class after the field trip.

    ***Parents/Guardians will confirm notification of this agreement by checking the required check box with the statement:
    I have reviewed the information provided about media and field trips. I understand I must contact the school office in writing if I do not wish to have my student participate.

    Pestcide
    Under Illinois law, public schools must notify parents/guardians and school employees at least 48 hours prior to any pesticide application on school property. The notification requirement extends to indoor and outdoor pesticide applications. Excluded from the notification requirement are antimicrobial agents (such as disinfectants, sanitizers, or deodorizers), insecticide baits, and rodenticides baits. Placement on this registry will provide a 48-hour prior notification of the application unless it was an emergency application.

    I have reviewed the information provided about pesticide notification. I understand I must contact the school office in writing if I wish to have my name added to the notification registry.

All Students - Release Agreement - Directory Release

  • The law and Board Policy designate certain information as “Directory Information.” Directory Information consists of identifying information, including the student’s name, address, gender, grade level, birthdate and birthplace, academic awards and honors, information relating to school-sponsored activities, organizations, and athletics, period of attendance in the school, and his/her parent’s names, addresses and phone numbers.. School district personnel may release "directory information" concerning students to members of the general public upon request for a variety of purposes such as yearbooks, honors and awards articles in the media. Parents who request that directory information not be released should notify the school on an annual basis before the beginning of the school year.

    ***Parents/Guardians will confirm notification of this agreement by checking the required check box with the statement:
    I understand I must contact the school office in writing if I do not want my student's directory information to be included in school publications that may be released to the public.

All Students - Computer and Internet Usage

  • Student use of a computer hardware and software in school is a privilege and is encouraged. Acceptable uses of computer hardware and software are defined in Board Policy 6:235, and students must utilize all technology, including Internet access, in a responsible and acceptable manner. Failure to do so could result in disciplinary action and/or loss of all privileges. Students have no expectation of privacy in any material that is stored, transmitted, or received via the District’s electronic network or District computers, and students’ use of school computers may be monitored by school staff or administrators. Access to inappropriate Internet content will be restricted through filtering software designed for that purpose.

    In the online registration application, the agreement that parents sign-off on will be grade dependent. Please review the following documents where appropriate:
